Just based on some simple diagnostic exclusions, I'd say that the PDF 
problem with the collapsed metrics of Everson Mono indicates a font 
problem of some kind, while the other problems are related to Mac OT 

I don't have all the fonts in your test document, but those I do have 
are displaying as I would expect in Firefox under Windows Vista. I 
believe Firefox on Windows takes advantage of the Uniscribe system .dll 
to render Unicode text, so unsurprisingly the best results are the 
OpenType fonts with GPOS mark positioning: Ezra SIL SR and my SBL Hebrew 
font (excepting that these follow the Hebrew convention for low patah 
even on double-yod).

Since Everson Mono uses only GSUB OpenType lookups, not dynamic mark 
positioning, only some of the vocalised letters display correctly. Those 
with hiriq do not (see attached screenshot).

There is definitely a problem with the Everson Mono font and Acrobat. I 
tried printing your test html document to PDF from Firefox in Windows, 
but Distiller flushed the job:

Michael, I'll run Everson Mono through MS Font Validator and see if I 
can identify any problems likely to be causing this problem.
